For your convenience and to help expedite the processing of new patients, we have provided online forms to be completed prior to your first appointment. Once you have finished filling out the forms, please hit the submit button (on the last page) to transmit the information to our office.  If for whatever reason, you are unable to electronically submit the form, you can always print the form and bring it with you to your CFD appointment.

Insurance Information

Patients - please bring all applicable insurance cards with you for your appointment.  Please also be prepared to pay
any co-pay owing at the time of your appointment.  We accept cash, checks, Visa, and Mastercard.
